Treatment of Mandibular Condyle Fracture by Monocortical Osteosynthesis after Oblique Mandibular Osteotomy


Abstract
The operation for the reduction and fixation of mandibular condyle fracture is a formidable procedilre frequently attended by unexpected complications because of the deep position of mandibular condyle, the proximity of the branches of the seventh nerve and the internal maxillary artery, and the strong action of the external pterygoid muscle causing displacement.
The author had treated a patient 28-year-old male, with right side mandibular condyle process fracture with severe opening limitation of mouth, masticational inability and malalignment of teeth at department of dentistry of Hanyang University Infirmary.
For open reduction and fixation, the oblique osteotomy from sigmoid notch to angle at the affected mandible was performed firstly, and secondly the postelior part of ramus and fractured segment were removed carefully to outside of the body.
After the fixation by the monocortical miniplate and screw of two detached fragments, the united bone was replanted at the original anatomic site.
According to the operation of this method, the author found out short period of intermaxillary fixation, rapidly recovery of mastication function and low side effects due to intermaxillary fixation.
